Require insurance companies to separate the premiums for the different components of health insurance akin to the way they separate comprehensive insurance, collision insurance, maintenance, prevention, etc. related to car insurance, warranties, and maintenance plans. This will radically improve transparency regarding which components of health insurance are expensive, allow health policy discussions about how to reduce costs of each element, and allow flexibility for individual choices for each element.
